Please note the following instructions:

 

• Do not sign or date documents prior to appointment, we will properly identify you and witness you signing the document. 

 

• Complete all your documents prior to the appointment except for signature and dates.

 

• The person signing the document must appear at the appointment.

 

• To verify your identity, Government issued photo identification is required (Driver's License, Passport, Citizenship Card).

• Please note we do not Notarize any documents

A commissioner does not have to know the person making the affidavit.  A commissioner administers the oath or affirmation that the affidavit is true.  The commissioner then signs the document to verify he/she saw the document being signed and he/she administered the oath.
